Inequalities in Girls’ High School Sports Participation: How Social Class, Race/Ethnicity, and Gender Route Opportunities to Play and Persist in Athletics

ABSTRACT High school athletics are understudied sites for social and educational stratification. Participation can offer mental and physical health benefits, improved student retention and graduation rates, the encouragement of pro-social behaviors, resume building, and enhanced social statuses. Despite legal prohibitions against race and gender discrimination in schools, opportunities to play and persist in interscholastic athletics may reflect and amplify existing social and educational stratification processes. Using an exploratory sequential mixed-methods research design, this study centers on girls’ high school sports and considers how gender, race/ethnicity, and social class operate at the individual, interactional, cultural, and institutional levels and encourage proclivities, commitments, and support for participation. We combine qualitative (N = 28 women and 47 total college athletes) and quantitative (N = 4,271 high school students) studies to inquire about how, and to what extent, racial/ethnic and social-class dynamics affect girls playing any and specific high school sports and whether they play persistently. Findings suggest that schools co-construct unequal athletic opportunity structures by nurturing and rewarding a cultivated athletic habitus associated with masculinity, whiteness, and affluent dispositions. These processes disguise athletic advantages and successes as well-earned merit and restrict who is most likely to receive the individual and social benefits of high school sports participation.

Opioids and Youth Athletes.

The ongoing opioid epidemic and associated adverse effects impart a large burden on our current healthcare system. The annual economic and noneconomic cost of opioid use disorder and fatal opioid overdose is currently estimated at $1 trillion. This review presents the prevalence, frequency of use, need, and effectiveness of opioid analgesia in the youth and adolescent athlete population. It identifies current indications for opioid versus nonopioid analgesic use in the setting of acute orthopaedic injuries, postoperative management, concussion, and chronic pain. Current knowledge of youth athlete opioid use, risks related to use, misuse, diversion, and addiction are reviewed. A PubMed, Medline, and Cochrane Library search was conducted in February 2023 to review opioid pain management strategies in the pediatric athlete population from 2000 to present. Searches were restricted to English language articles and human subjects. Initial reviews of titles and abstracts were performed by all authors and relevant full-text articles were selected. Priority was given to systematic and narrative reviews, meta-analyses, and prospective studies. Narrative review. Level 3. First author name, publication year, study design, study country, subject demographics, and data on the frequency, type, and duration of analgesic treatments for musculoskeletal injuries, postsurgical care, chronic pain disorders, and concussion were extracted. Pediatric athletes comprise a high-risk population seeking analgesic relief for injury-related pain. Participation in high school sports is associated with increased risk of opioid use. An average of 28% to 46% of high school athletes have used opioids in their lifetime. Participation in ≥1 high school sport puts adolescents at 30% greater odds of future opioid misuse. The use of opioids in the pediatric athlete population is common and associated with both short- and long-term risks of misuse and addiction.

Differences in Overuse Injuries in Gender-Comparable Sports: A Nationally Representative Sample of High School Athletes.

Participation in high school sports has physical, physiological, and social development benefits, while increasing the risk of acute and overuse injuries. Risk of sports-related overuse injury differs between boys and girls. To investigate differences in overuse injuries among United States high school athletes participating in the gender-comparable sports of soccer, basketball, and baseball/softball. Descriptive epidemiology study using a nationally representative sample from the High School Reporting Information Online (RIOTM) database. High schools. Athletes with overuse injuries during the 2006-2007 through 2018-2019 academic years. National estimates and rates of overuse injuries were extrapolated from weighted observed numbers (with the independent variables: sport, gender, academic year, class year, event type, body site, diagnosis, recurrence, activity, and position). Among an estimated 908, 295 overuse injuries nationally, 43.9% (n=398,419) occurred in boys' soccer, basketball, and baseball, while 56.1% (n=509,876) occurred in girls' soccer, basketball, and softball. When comparing gender across sports, girls were more likely to sustain an overuse injury than boys (soccer: IRR: 1.37; 95% CI: 1.20-1.57; basketball: IRR: 1.82; 95% CI: 1.56-2.14; and baseball/softball: IRR: 1.21; 95% CI: 1.04-1.41). Most overuse injuries in soccer and basketball for both genders occurred to a lower extremity (soccer: 83.9% [175,369/209,071] for boys, 90.0% [243,879/271,092] for girls; basketball: 77.0% [59,239/76,884] for boys, 80.5% [81,826/101,709] for girls), while most overuse injuries in baseball and softball were to an upper extremity (72.5% [81,363/112,213] for boys, 53.7% [73,557/136,990] for girls). For boys' baseball, pitching (43.5% [47,007/107,984]) was the most common activity associated with an overuse injury, which differed from the most common activity of throwing (31.7% [39,921/126,104]) for girls' softball. Gender differences observed in this study can help guide future strategies that are more specific to gender and sport to reduce overuse injuries among high school athletes.

The culture of sport: examining the effect of athletic participation on the acceptance of rape myths

PurposeThe purpose of this paper is to examine the relationship between athletic participation and the acceptance of rape myths in an effort to further identify cultural and social institutions that may contribute to adherence to and glorification of rape culture.Design/methodology/approachA random sample of 685 first semester, university freshmen were surveyed about their high school sports participation and levels of rape myth acceptance (RMA). Linear regression models were estimated to examine the relationships between participation in sports, the type of sport participation and the acceptance of rape myths.FindingsThose involved in athletics, generally, and those involved in team sports are more accepting of rape myths than are their counterparts. These results hold true for the full sample and for males and females, when examined separately. Participation in contact sports was not significantly predictive of RMA.Practical implicationsAthletics may provide a culture that is particularly prone to a belief in rape myths, which jeopardizes the integrity of collegiate sports. Policies should focus on changing offender behavior not victim behavior to create safer and more inclusive communities. Educating youth about the nature of rape myths and providing them with skills to resist such thinking is paramount. Focused programs and training could reduce the likelihood of accepting rape myths and, in turn, may make high school and college campuses safer.Originality/valueThis paper contributes to prior research by examining a sample of first semester university freshmen about their high school athletic participation and RMA. This research not only minimizes the effects of college level influences but also distinguishes between different forms of athletic involvement.

Sport Participation, Extracurricular Activity Involvement, and Psychological Distress: A Latent Class Analysis of Canadian High School Student-Athletes

Although psychological distress has been shown to increase during adolescence, participation in organized activities may have protective effects. The present study aimed to identify whether there is a relationship between high school student-athletes’ breadth of participation in organized activities and psychological distress, using a latent class analysis. Canadian adolescent-athletes (n = 930) in Grades 11 and 12 completed an online survey that measured: (a) high school sport participation, (b) community sport participation, (c) nonsport extracurricular activities participation, and (d) psychological distress. The latent class analysis indicated that a two-class model (i.e., Class 1 = narrower breadth, low distress; Class 2 = wider breadth, moderate distress) was most appropriate. Results indicated that despite the divergent probability of organized activity participation, participants in both classes had a low to moderate probability of presenting elevated levels of psychological distress. However, levels of psychological distress were still higher than other Canadian adolescent populations, suggesting that overscheduling could be of concern. Gender and time (i.e., prior/during COVID-19 pandemic) were significant covariates in the model.

Youth sport participation, injury history, and current physical activity among young adults

Objective: To examine the interaction between high school (HS) sports participation and injury history with current moderate-to-vigorous physical activity (MVPA) among young adults. Participants: Participants (N = 236) were 18–25 years old, not currently injured, and reported no physical activity limitations. Methods: Participants completed online demographic, injury history, and physical activity surveys. A two-way analysis of covariance was used to test the interaction between HS athlete status and previous injury severity on current self-reported MVPA. Results: Participants were 22.2 ± 2.1 years, primarily White (81.8%) or Asian (6.4%), and female (77.5%). After including body mass index and race as covariates, there was a statistically significant interaction between HS athlete status and previous injury history such that current MVPA was higher among former HS athletes compared to HS recreational/nonathletes when individuals reported no injuries or mild injury severity. MVPA was similar across athlete status groups when participants reported high levels of injury severity. Conclusions: Future studies should examine whether young adults who have experienced multiple and/or severe injuries as competitive HS athletes have unique physical activity barriers.

Relationships between personality traits, high school sports participation, and physical activity of college students in the United States

Objective: The study aimed to 1) examine relationships between contexts of PA, personality traits, and high school sports participation (sports) and 2) determine significant PA correlates in a college population. Participants: 237 undergraduates at a university in the United States between September 2020 and May 2021 participated. Methods: Participants completed a survey evaluating PA, personality traits, sports, and demographic variables. Pearson partial correlations assessed correlations between different PA domains, personality traits, and sports. Conscientiousness was positively associated with all PA measures (r = .14-.30), except for active transport PA. Sports were related to vigorous and leisure PA. Conscientiousness is related to PA measures and is a significant PA correlate. However, more research is necessary to understand if leisure time PA can enhance Conscientiousness.

The role of sport participation on exercise self-efficacy, psychological need satisfaction, and resilience among college freshmen

Objective: To evaluate the relationship between high school sport participation among university students and whether previous sports participation was associated with current levels of exercise-self efficacy, resilience, and psychological need satisfaction physical activity behaviors. Participants: College freshmen (n = 152) in the United States responded to an electronic survey sent out by their university. Methods: Participants completed a set of questionnaires assessing resilience, psychological need satisfaction in exercise, self-efficacy, and current PA. Results: Analysis of variance, Kruskal-Wallis H tests, independent samples t-tests, and ordinary least squares regression modeling were used. Total PA and having met moderate PA guidelines were both associated with exercise SE and PNSE. HS GPA, total PA, and being a college athlete were associated with both resilience and exercise SE. Conclusions: Sport participation may serve as an effective method for students to develop exercise SE, resilience, and PNSE. The development of these skills may support lifelong PA behaviors.

Psychological Experiences During Previous High School Sport Participation Predict College Students’ Current Psychological Health

Adolescent sport participation has been positively associated with psychological health outcomes. Yet, further research is needed to explore how psychosocial health benefits from sport may be maximized or minimized based on one’s psychological experiences during previous sport participation. The present study examined associations among retrospective psychological experiences of high school sport participation and markers of current college students’ psychosocial health. American college students (N = 300) self-reported retrospective high school sport experiences (i.e., burnout, engagement, and stress) and current psychosocial health outcomes (i.e., social support, depressive symptoms, life satisfaction) via an online interface. Moderated multiple regression analyses showed high school sport burnout, stress, and engagement to predict significant variance in college social support (p < .05), anxiety (p < .01), depressive symptoms (p < .01), and life satisfaction (p < .01). Results provide evidence that retrospective accounts of high school sport participation experiences (i.e., low burnout, low stress, and positive engagement) were associated with more adaptive post-high school psychosocial outcomes in college. This information may guide future prospective studies and aid practitioners by proving a broader understanding of psychosocial outcomes of sport participation.

Subsequent Musculoskeletal Injury Incidence Are Similar Between General Infection and COVID-19 Among High School Athletes in the Unites States Despite Higher Illness Incidence During Academic Years 2019-2020 and 2020-2021.

To describe overall illness and COVID-19- specific illness in high school athletes in the 2019-2020 and 2020-2021 academic school years, and to describe and assess the risk of musculoskeletal injury following general infection and after COVID-19. Ecological study. High schools (6 states; 176 high schools) were matched between the 2019-2020 and 2020-2021 academic school years, based on 2020-2021 high school sport participation. Illness and injury data were collected from the high school athletic trainers. Illness was stratified by overall illness, general infection, and COVID-19. Injuries following moderate or severe infections or COVID-19 were recorded. Illness and injury incidence rate per 100 athletes per year, with 95% confidence intervals (95% CIs), were calculated. Negative binomial models comparing injury following general infections and COVID-19 infections were calculated. A total of 98 487 and 72 521 athletes participated in the 2019-2020 and 2020-2021 years. Illness incidence rate was less in the 2019-2020 academic school year [0.30 (95% CI: 0.27, 0.34)] than the 2020-2021 academic school year [1.1 (1.0-1.2)], resulting in a difference of 0.8 (95% CI: 0.7, 0.9). COVID-19 incidence rate was 0.52 (0.47-0.58) in the 2020-2021 year. Injury following general infection incidence rate was 27.9 injuries (21.4-34.5) per 100 athletes in 2019-2020, and 22.5 injuries (19.3-25.7) per 100 athletes in 2020-2021. There was no difference in injury risk following general infection and COVID-19 [rate ratio: 1.2 (95% CI: 0.7, 2.4)]. The incidence rate for all illnesses in high school athletes was slightly (0.8) greater in the 2020-2021 academic compared to the 2019-2020 year. Most of the incidence increase was due to infections and COVID-19. Sleep habits of high school student-athletes and nonathletes during a semester.

Lack of sleep has been shown to be harmful to athletic and academic performance as well as health and well-being. The primary purpose of this study was to analyze the sleep and physical activity differences between US high school student-athletes and nonathletes during a semester of school and competition. Participants included 34 student-athletes (18 males and 16 females), age 15.8 ± 0.8 years, and 38 nonathletes (10 males and 28 females), age 16.3 ± 0.7 years. Objective sleep and physical activity outcomes were collected using Fitbit wrist-worn activity trackers for 8-14 consecutive days and nights, measuring total sleep time, sleep efficiency, bedtimes, wake times, and steps counted. Student-athletes and nonathletes did not differ in total sleep time (440.4 ± 46.4 vs 438.1 ± 41.7 min, P = .82) and sleep efficiency (93.6 ± 2.3 vs 92.9 ± 2.3%, P = .20). Fitbit data revealed that 79% of student-athletes and 87% of nonathletes failed to get greater than the minimally recommended 8 hours of total sleep time per night. Student-athletes had significantly more steps per day (10,163 ± 2,035 vs 8,418 ± 2,489, P < .01). Student-athletes had earlier bedtimes and wake times. Earlier bedtimes were significantly correlated with increased total sleep time (P < .01). Earlier wake times were significantly correlated to increased steps per day (P < .01). Participation in high school sports may not have a detrimental effect on a student's sleep habits. High school students are not meeting the recommended 8-10 hours of sleep per night. Going to bed and waking up early were linked to healthier outcomes. Consistent and earlier sleep/wake schedules may optimize students sleep and health. Ungaro CT, De Chavez PJD. The Making of a College Athlete: High School Experiences, Socioeconomic Advantages, and the Likelihood of Playing College Sports

Understandings of who plays college sports are dominated by assumptions that lack academic scrutiny. Using the Education Longitudinal Study (N = 7,810) and multilevel modeling, this study examines the extent to which high school indicators of family socioeconomic statuses, athletic development and merit, academic expectations and knowledge, and school contexts predict the likelihood of becoming a college athlete. The authors find evidence that supports our understanding of the process of becoming a college athlete being shaped by family socioeconomic status. Still, high school sport participation characteristics, academic expectations and knowledge, and school contexts also seem to offer independent contributions to the odds of becoming a college athlete. Overall, these results suggest that college athletic opportunities are not simply a function of athletic merit, based on unique analyses of quantitative empirical evidence from a large national sample of high school students.

Feasibility of electrocardiogram screening in the USA prior to high school sport participation

BackgroundSudden cardiac death in young athletes is a catastrophic event with an incidence of 1:40,000–1:80,000. Pre-participation sport screening utilizes a 14-point targeted evaluation. Recently, some states have added an ECG to the traditional history and physical. ObjectivesThe study describes a large-scale ECG screening program for middle and high school students. The aim is to depict important components, partners, and plans that can be adopted by other municipalities to help prevent sudden cardiac arrest in this population. MethodsIn 2019, Brevard County School Board in Florida mandated ECGs before participation in middle and high school sports. They partnered with a non-profit group that provides ECGs for a $20.00 fee. Volunteer pediatric cardiologists interpreted the ECGs. Results were emailed to participants, and abnormal results received a phone call to ensure appropriate follow-up care. ResultsThere were 5877 ECG screenings between 4/1/19–12/31/19 with 3% (199/5877) abnormal: 30% (60/199) ventricular conduction, 26% (51/199) arrhythmias, 13% (25/199) ventricular hypertrophy/chamber enlargement, 11% (21/199) ST-T abnormalities, 8% (16/199) prolonged QTc, 6% (12/199) WPW syndrome, and 4% (7/199) AV conduction and axis deviation. The incidence of critical heart disease with a risk of sudden cardiac arrest was 0.1% (8/5877) in those with a confirmed final diagnosis: 6 who underwent an ablation procedure for WPW syndrome, 1 with gene-positive long QTc, and 1 with left ventricular hypertrophy who is undergoing genetic evaluation for hypertrophic cardiomyopathy. ConclusionsTo our knowledge, this is the first large school district in Florida to mandate this level of cardiovascular screening. This pilot project demonstrates that large volume ECG screening prior to school sport participation is feasible at a low cost when utilizing appropriate manpower and providing access to follow up-care.

The Dice Are Loaded: Schools’ Social Class Composition and Athletic Contests

Research shows that social class differences in high school sports participation are large and growing. However, focusing on sports participation may obfuscate large social class differences in sports performance among participants. The authors develop theoretical predictions on the basis of exclusion (middle-class youth perform sports better) and inclusion (working-class youth perform sports better). To test these predictions, the authors analyze the relationship between high schools’ social class composition and success in high school athletics using data on more than 200,000 contests in school fixed-effects models. The findings indicate that predominantly middle-class schools beat economically integrated and predominantly working-class schools by large margins, supporting exclusivity perspectives. Also, predominantly working-class schools win as much as economically integrated schools, providing evidence of inclusion, but inclusion is much weaker than exclusion. The authors conclude that sports performance among youth is highly stratified by social class.

Employing Google Trends to Assess Concussion Search Popularity, Seasonality, and Association With High School Sports Participation.

This investigation employs 2004-2020 Google Trends data to correlate concussion search popularity and high school sports participation while assessing search seasonality and factors associated with search escalation. Events linked to concussions, from news to entertainment, altered public perception of concussions and discouraged high-risk activities. Such sentiment likely promoted high school football's 2004-2018 8% decrease in participation, despite simultaneous 8% increase for sports overall. Google Trends (GT) analysis calculates distinct subjects' popularity scores (0-100) by normalizing Google search frequency with other subjects in the same topic. With 35% of patients researching their conditions using Google, tracking concussion popularity scores allows robust analysis of event-driven fluctuation, seasonal variation, and sports impact. Worldwide GT analysis was conducted for the disease topic "Concussion" with the categorical filter "Health" from 2004 to 2020. This period's monthly popularity scores with annual averages and annual high school sports participation data were subsequently obtained from Google Trends and the National Federation of State High School Associations, respectively. Concurrence of significant score changes with CTE studies' publication, release of 2015 film Concussion, and the COVID-19 pandemic validates GT's measurement of public interest. From 2006 to 2016, Pearson's correlation coefficients demonstrated strong negative correlation between GT popularity scores for "Concussion" with total number of high school football participants overall (R2 = 0.8553) and participants per school (R2 = 0.9533). Confirming football-related seasonality, one-way ANOVA regression analysis concluded 2004-2020's mean change in month-specific popularity score is not the same (p = 3.193E-08), and months during football season had statistically significant variability (September: p = 4.389E-05) with elevated average z-scores. Coupled with ANOVA regression and normalized analyses, strong negative correlations between concussion popularity score and high school football participation produce quantitative measures of a long-suspected relationship. Tracking concussion popularity and seasonality with Google Trends provides insight into how, when, and why patients are educating themselves online.

Reported COVID-19 Incidence in Indoor Winter Sports Among US High-School Athletes.

To evaluate the association between COVID-19 incidence among high-school athletes participating in different indoor winter sports and attending schools with in-person versus virtual instructional delivery. Cross-sectional survey. US high schools. High-school athletic directors. Indoor winter sports, school instructional delivery method, and state COVID-19 incidence. Surveys were distributed to high-school athletic directors throughout the United States regarding sport reinitiation and COVID-19 cases in winter 2020 to 2021. Separate mixed effects Poisson regression models were developed to evaluate the associations between reported COVID-19 incidence and (1) different sports and (2) school instructional delivery method, while adjusting for the background, state COVID-19 incidence. Four hundred thirty schools had restarted fall sports, representing 31 274 athletes on 1404 teams from 14 states. One thousand four hundred sixty cases of COVID-19 were reported, representing a case rate of 4668 cases per 100 000 athletes and an incidence rate of 58.7 cases per 100 000 player-days. Reported COVID-19 incidence was greatest among girls' hockey (82.1 cases per 100 000 player-days (95% CI, 56.8-115) and boys' hockey [76.7 (61.8-94.2)] and lowest among swimming [39.0 (31.1-48.2)] and gymnastics [28.5 (13.1-54.2)]. No difference in reported COVID-19 incidence was identified among athletes attending schools with virtual versus in-person instruction [incidence rate ratio = 0.86 (0.52-1.4)]. In this nationwide survey of US high-school athletic directors regarding indoor winter sports, reported COVID-19 incidence was lower in noncontact sports but unrelated to school instructional delivery method. This information may help guide decision-making regarding high-school sport participation during the COVID-19 pandemic.
